their healthcare costs. A simple blood test is necessary for prevention or early
detection of disease and could save your life. Your lab program provides you a
savings of 20 to 50% off regular retail price on blood tests available at over
5,000 Clinical Laboratory Improvement Amendment certified facilities nationwide.

Disease, Prostate Cancer, Diabetes, Thyroid Disease and more...
Diagnostic Imaging
Members have access to superior quality testing facilities for you and your
family to have MRI's, CT's, or P.E.T Scan's at significantly reduced prices. A
network of more than 3,900 highly credentialed testing facilities throughout the
United States enable us to offer you imaging test at considerably lower prices.
There are no hidden charges - you simply tell us what location of the country
you would like to go to for your diagnostic imaging test and exam and we will
schedule it for you at a convenient location and at a significantly reduced
cost. Our rates at our contracted radiology facilities are 20 to 70% off the
typical billed charges. Once your test is ordered, we will locate the nearest
testing center for you and we will send you all of the necessary paperwork. We
offer you the same high quality imaging test as your doctor and hospital, but at
deeply reduced rates.

Lab Services
- To schedule Lab Services click "Order Laboratory Services" button to the upper right.
- Locate a facility nearest to you and place your order online.

Diagnostic Services
- To schedule Diagnostic Services click button to the upper right.
- Place your order by calling the Toll Free number on your ID Card.
- A customer service representative will assist you in locating a facility
nearest you and will provide instructions on how to get set up.

Terms of Use
ID valid for two (2) weeks from date printed. However, with your free membership you may print a new
one as often as needed while membership is active. Prices may vary by
location. Valid at participating network providers only. Printed ID is
required to receive discount.
Disclosures
a. The discount medical card program is NOT health insurance.
b. The plan provides discounts at certain health care providers for medical
services.
c. The plan does not make payments directly to the providers of medical
services.
d. The range of discounts for medical or ancillary services provided
under the plan will vary depending on the type of provider and medical or
ancillary services received.
e. The plan member is obligated to pay for all health care services but
will receive a discount from those health care providers who have contracted
with VantageAmerica Solutions, Inc., a discount medical plan organization.

ATTENTION MARYLAND RESIDENTS
Some discounts under the Physician and Hospital Referral Plan benefit are
not applicable in Maryland. Discounts are not available for all In-Patient
Procedures and certain Out-Patient Procedures under Maryland law. Out-Patient
Procedures at network hospitals such as laboratory and diagnostics services are
eligible for the discount.
The program and its
administrators have no liability for providing or guaranteeing service by
providers or the quality of service rendered by providers. This program is not
available to residents of Alaska, Massachusetts, Minnesota, Montana, and
Vermont.
