• To locate an EyeMed provider, click the provider locator.
• Print the ID from this site (EyeMed provider locator page does not provide "Print ID" option) and present it, along with your photo ID, to the eye care provider and dispensary.
• The provider will apply the discount to your charges, and you'll save money while saving your sight!
  

Terms of Use
ID valid for two (2) weeks from date printed.  However, with your free membership you may print a new one as often as needed while membership is active.  Prices may vary by location.  Valid at participating network providers only.  Printed ID is required to receive discount.  Frames, lenses and lens options must be purchased in the same transaction to receive full discount.  The discount does not apply to the doctor’s professional services or disposable contact lenses.  The discount is not available for the following: Orthoptic or vision training, subnormal vision aids, and any associated supplemental testing; medical and/or surgical treatment of the eye, eyes, or supporting structures; services provided as a result of any Worker's Compensation law; frames where the manufacturer prohibits a discount. Not valid with any other special or promotion.

ATTENTION MARYLAND RESIDENTS
Some discounts under the Physician and Hospital Referral Plan benefit are not applicable in Maryland. Discounts are not available for all In-Patient Procedures and certain Out-Patient Procedures under Maryland law. Out-Patient Procedures at network hospitals such as laboratory and diagnostics services are eligible for the discount.

The program and its administrators have no liability for providing or guaranteeing service by providers or the quality of service rendered by providers. This program is not available to residents of Alaska, Massachusetts, Minnesota, Montana, and Vermont.
