- Join in the Telephone Counseling Program
at special member pricing.
- Call the 800 number listed on ID card.
- Provide Customer Service with the Group Name listed on the ID

But doesn't everyone have problems like those at some time?
Of course, and most people, by themselves or with the help of friends or family
can solve them. But sometimes problems can be of such size that this help simply
isn't enough. That's when we can help.

How does one get help?
We encourage voluntary participation. If a person thinks they have a problem, they call the number and speak with a licensed counselor.
You may be referred to a licensed mental health professional in your area.

Can family members participate?
Yes. Member's personal problems can affect the family. Therefore, members' families are encouraged to use the Counseling Service as needed.

Who will know if I use the Counseling Service?
Counseling is strictly confidential. Only the individual receiving the services can authorize the release of information.

Who will a person talk to?
A professional counselor will work directly and confidentially with the member or family member to help resolve the problem. In some cases,
the counselor's role will be to assist in determining what additional community resources can provide further assistance.

What kind of counseling is covered by my membership?
Phone counseling. The implied objective of phone counseling is the resolution of a problem within a brief period of time. As a result, phone counseling is not appropriate for each member referred to the Counseling Service. The phone counselor will weigh the nature of the problem, as well as the member's desire and ability to work on resolving the problem, and determine whether
referral to a face-to-face counselor is needed. This referral might be paid through the member's insurance.
Terms of Use
ID valid for two (2) weeks from date printed. You may print a new one as often as needed while membership is active. Prices may vary by location. Valid at participating network providers only. Printed ID is required to receive discount.
Disclosures
a. The
discount medical card program is NOT health insurance.
b.
The plan provides discounts at certain health care providers for medical services.
c.
The plan does not make payments directly to the providers of medical services.
d.
The range of discounts for medical or ancillary services provided under the plan
will vary depending on the type of provider and medical or ancillary services received.
e. The plan member is obligated to pay for all health care services but will receive a discount from those health care providers who have contracted with VantageAmerica
Solutions, Inc., a discount medical plan organization.

in Maryland. Discounts are not available for all In-Patient Procedures and certain
Out-Patient Procedures under Maryland law. Out-Patient Procedures at network hospitals
such as laboratory and diagnostics services are eligible for the discount.
The program and its
administrators have no liability for providing or guaranteeing service by
providers or the quality of service rendered by providers. This program is not
available in Montana, Vermont, Massachusetts, Alaska, Deleware, Minnesota, Kansas, Washington,
Florida, Nevada, Oklahoma, South Carolina, and Puerto Rico.
