Medical Information
LifeTrack, LLC functions in accordance with HIPAA and the HIPAA Privacy Rules. No
personally identifiable information is accessible by outsiders. No information is reproduced or reprinted at LifeTrack, LLC offices. All information that is submitted via
fax, mail, or email is destroyed or erased immediately after it is processed.
LifeTrack, LLC will not release personal or medical information to third parties,
except to expressly named physicians authorized by the client or if release is required
by law or is subpoenaed to judicial or governmental investigations or proceedings. There are no other circumstances under
which we will provide or sell personal information to third parties.
