The program uses networks of dentists to provide members and their family
an affordable solution to quality dental care. Members can save 10% - 50% on
quality dental care through a nationwide network of over 29,000 dental
providers. Savings include routine dental services such as cleanings, x-rays,
and fillings. Plus, you also can save on specialty care such as orthodontics,
oral surgery and periodontics where available.

NOTE: If you have a dental insurance plan that is not a PPO or
HMO and your dentist is a participating provider, you may be able to use your
membership card to receive additional savings.

Instructions
It's as easy as 1-2-3 to start improving your Dental Care.
- To locate a provider, click on the
provider locator. When calling a dentist for an appointment, identify yourself
as a member with access to the Uni-Care Network.
- Print the ID from this site and
present it, along with your photo ID, to the provider.
- The provider will apply the discount to your charges. You are responsible for
the total bill, less the applicable savings, at the time service is rendered.
You will save money while saving your oral health!
- While our provider list are continually updated, provider status can change. We
recommend that you confirm the provider you selected participates in the program
when scheduling your appointment.
Terms of Use Terms of Use
ID valid for two (2) weeks from date printed. However, with your free membership
you may print a new one as often as needed while membership is active. Prices
may vary by location. Valid at participating network providers only. Printed ID
is required to receive discount.
Disclosures
a. The discount medical card program is NOT health insurance.
b. The plan provides discounts at certain health care providers for medical
services.
c. The plan does not make payments directly to the providers of medical
services.
d. The range of discounts for medical or ancillary services provided
under the plan will vary depending on the type of provider and medical or
ancillary services received.
e. The plan member is obligated to pay for all health care services but
will receive a discount from those health care providers who have contracted
with VantageAmerica Solutions, Inc., a discount medical plan organization.

ATTENTION MARYLAND RESIDENTS
Some discounts under the PhysicianSome discounts under the Physician and
Hospital Plan benefit are not applicable in Maryland. Discounts are not
available for all In-Patient Procedures and certain Out-Patient Procedures under
Maryland law. Out-Patient Procedures at network hospitals such as laboratory and
diagnostics services are eligible for the discount.
The program and its administrators have no liability for providing or
guaranteeing service by providers or the quality of service rendered by
providers. This program is not available to residents of Alaska, Massachusetts,
Minnesota, Montana, and Vermont.
